    The coordinator will conduct a home care assessment that includes a walk-through around the home to look for safety hazards and fall risks.

    The coordinator will use the information to create a personalized care plan. Together, we’ll make sure the plan is comprehensive, flexible, and respectful of personal preferences.

    Your care coordinator will consider your input and information about your loved one's life, hobbies, preferences, and health conditions. Then, we’ll find a caregiver who has the skills, experience, personality, and availability to foster a perfect match.

    If — for any reason, at any time — you feel the caregiver isn’t the right match, the care coordinator will work with you to select a different home care aide.
